Hours: Full-time, Monday – Friday, 09:00 – 17:00
Pay Rate: £19.05 (PAYE + holiday) or £22.00 umbrella/ hour

Home First South are looking for 2 x Band 5 Community Nurses (RGN) to join their supportive nursing team ASAP. This is a fantastic opportunity for registered nurses who are passionate about community healthcare and want to provide high-quality, patient-focused care across Staffordshire.

About the Role

As a Community Nurse (Band 5 RGN), you will:

• Deliver nursing care to patients in their own homes, residential homes, and local clinics

• Assess, plan, implement, and evaluate patient care packages

• Support the prevention of unnecessary hospital admissions

• Work closely with multi-disciplinary teams to improve patient outcomes

• Promote independence, wellbeing, and self-management

Requirements

• NMC Registered Nurse (RGN)

• Experience in Community Nursing and/or District Nursing

• Excellent assessment and care planning skills

• Ability to work independently and as part of a team

• A car driver is essential (travel required across localities)
