Your Role as a Community Nurse Includes:

You’ll be providing person-centred care in patients’ homes or local clinics, supporting individuals with long-term conditions, post-discharge care, palliative needs, and more.

Key skills and clinical duties include:

• Female catheterisation

• Male catheterisation

• Syringe driver management

• Central line care

• PR medication administration (enemas and suppositories)

• Catheter maintenance and bladder scanning

• Pressure ulcer prevention and wound care (simple to complex, including packing wounds)

• Diabetes management: CBG monitoring, insulin administration, and patient education

• Administering medications via injections and PEG

• End of Life care, including PRN injections

• SystmOne: care plan creation, tasking, referrals, discharges, and visit amendments

Who You Are:

• A Registered Nurse (NMC Pin)

• At least 6 months’ experience in UK community nursing

• Confident working autonomously and managing a varied caseload

• Skilled in female catheterisation (essential)

• Comfortable using SystmOne for documentation and patient management

• Compassionate, organised, and proactive in your approach to care
